NS5a Inhibitors
NS5a inhibitors may be prescribed with pegylated interferon and
Ribavirin, or as part of an all oral treatment (treatment
without interferon). This can interfere at several stages of the
lifecycle of the Hepatitis C virus. The following NS5a inhibitors
are available in Scotland:
Daclatasvir
What is it and who is it for?
Daclatasvir is an oral medication prescribed with other
Hepatitis C medications. It is a pill taken once daily. It is
effective across all genotypes but in Scoltand is it restricted for
use only in those with high fibrosis scores (F3 or F4) or comensated
cirrhosis.
How long does treatment take?
If you have genotype 1 or 4 with no cirrhosis, but have a
fibrosis score of F3 or F4, you'd be treated with daclatasvir and
sofosbuvir for 12 weeks. If you have genotype 1 or 4 but have compensated
cirrhosis, you'd be treated with for 24 weeks and may also be
prescribed ribavirin.
If you have genotype 3 with compensated cirrhosis you'd be
treated with daclatasvir and sofosbuvir alongside ribavirin for 24
weeks.
If you have genotype 4 you'd be treated with daclatasvir for 24
weeks plus pegylated interferon and
ribavirin for either 24 or 48 weeks.
Genotype 1 or 4 without cirrhosis
: daclatasvir + sofosbuvir for 12 weeks.
Genotype 1 or 4 with compensated cirrhosis
:
daclatasvir + sofosbuvir for 24 weeks (consider add
ing
ribavirin for patients with very advanced liver dis
ease or with other negative prognostic factors such
as prior treatment experience).
Genotype 3 with compensated cirrhosis and/or treatm
ent experienced
:
daclatasvir + sofosbuvir +
ribavirin for 24 weeks.
Genotype 4
: daclatasvir for 24 weeks + peginterferon alfa + r
ibavirin for 24 to 48 weeks.
